Starting May 1st through the end of school, the clinic is offering free sports physical examinations. Clinic hours will be Monday, Wednesday, and Friday from 7:30am to 3:00pm. If you would like for your child to have an examination, please complete the WVSSAC Athletic Participation/Parental Consent/Physician’s Certificate Form (please complete the form including the medical history and be sure to sign the forms) and you and/or your child may bring it to the clinic at the time of the exam.
